We get it. Things come up at the last minute. But to protect our practice members’ and providers’ time, we require a deposit for reserving a new patient consultation and notification for rescheduling or canceling appointments. We understand that unforeseen circumstances may arise… we just asking that you let us know. Please understand that last-minute changes affect other practice members, as well.
A non-refundable $60 deposit is required to reserve a new patient appointment. The deposit is used as a guarantee for your appointment. Should an examination be warranted, the deposit will be applied toward any cost incurred for your visit OR the deposit will be carried over for use at future visits and deducted from the total cost of any services rendered, provided you meet the cancellation and reschedule policy requirements.
As the initial consultation is complimentary, should an examination not be warranted, or you choose not to move forward with an examination, the deposit will be fully refunded to you.
Should you not show for your appointment OR cancel/reschedule without 24-hour notice, your deposit will be forfeited.
If you need to cancel or reschedule you can do so via text, email, or phone.
Special Report Appointments: We require 24-hour notice for rescheduling or canceling a special report appointment (i.e.. Initial Review of Results, Review of Corrective Care, Review of Rebuilding Care, etc.).
Should you not show for your appointment OR cancel/reschedule without 24-hour notice, you may be charged a $30 late cancellation fee to the card on file.
If you need to cancel or reschedule you can do so via text, email, or phone.
